I got every required question right, but the quiz still says I didn't pass — why?

Passing requires every auto-gradeable question to be correct, not just the required ones. A non-required multiple choice/true-false question isn't locked, so it's easy to leave one wrong without noticing — check for a red-flagged question further up and fix it, then resubmit.
